I wonder if this can be submitted in addition to the other Pepsi promo on the spendandsave.ca This one seems to be strictly Loblaws stores for gc and the other is for a pepsi coupon booklet....

Possibly... I have both addy`s here and they have different PO boxes and Postal codes!

On both forms it says it needs to be the ORIGINAL proof of purchase (UPC)

For the coupon booklet offer, yes you can print it from the site - that may be the only way of getting one, actually. I've seen a display for this offer at Food Basics, but the coupon pad had only a piece of paper saying to get the form from the website instead of forms.

For the gift card offer, the form can be found in stores. I haven't seen a printable form.

Check at your local Loblaws company store like Zehrs/Superstore/Fortinos etc for a big Superbowl display - that's where you can find a form. At my local Zehrs and RCSS the displays were near the entrance to the store.
