The Centre for Contact Lens Research at the University of Waterloo is interested in hearing about your contact lens-wearing experiences. An online survey is being conducted by Drs. Desmond Fonn, Lyndon Jones, Craig Woods and Kathy Dumbleton. The purpose of the survey is:

To find how many Canadian contact lens wearers either temporarily or permanently stopped wearing contact lenses over the last two years;
To understand the reasons people stop wearing contact lenses;
To understand the reasons people start wearing their contact lenses again after a significant break from wear.
The survey will take approximately 10 minutes to complete. We’re interested in hearing from you about:

the type of lenses you are wearing (or used to wear)
your lens wearing patterns
your contact lens care habits
We are looking for 3000 contact lens wearers and 1500 lapsed contact lens wearers. Your eligibility will be determined in the first few questions on the survey.

To respond to the survey, click on the button at the bottom of this page. It is important that you read the following information first:

Responding to this survey is voluntary. You may decline to answer any questions you do not wish to answer, and you can quit at any time by closing your browser.
Any information that you provide will be kept confidential, in a secure location within the School of Optometry. The survey software does not keep track of IP addresses (the location of your computer).
The information collected in this study will be electronically archived in a secure database for a minimum of 25 years.
Is there remuneration for responding to this survey? We will send you a $10 Tim Horton’s gift card in return for your time, if you submit your name and address at the end of the survey. This information will be kept separate from your survey responses and will not be used for any other purpose or provided to any other organizations. More details will be provided at the end of the survey.

This survey has been reviewed and received ethics clearance through the Office of Research Ethics at the University of Waterloo. However, the final decision about participation is yours.
