It came on a postcard like insert in my bag when I picked up my prescriptions from Shoppers I am going to attempt to add a picture of it..Yikes I am terrible at this kind of stuff....

Sorry guys! I managed to scan it but have no idea how to attach it so that you can all see it...will keep trying. In the meantime if anyone has an email address they are willing to share and I can send it to them and they can post it on here that would be awesome!!
