Quaker Harvest Crunch Quinoa Chocolate Granola Bars Rebate Form

After not picking up the rebate coupon when I saw it on the wall at Superstore, I finally found the bars at Walmart, and bought at full price thinking I would pick up the form next time I was in Superstore, but it's gone. Did anyone manage to pick up an extra of these, or have you seen them at other stores recently around town?
