It says you are supposed to include the original receipt in separate envelopes because they go to different PO Boxes however I am sure lots of us did it differently

For myself I purchased all of them on one receipt at SDM and asked SDM to reprint my original receipt

I kept the reprinted receipt to keep track of which rebates I had done with which house as it is 1 per household of each upc - as I got the cheques in as they come all separately I highlighted them on my receipt

The original did go to PF but if I bought 4 items, I would have included 3 photocopies of the original with a note stating that the original receipt was already handed in for xx rebate

Sent ALL rebates to them in separate envelopes with the stickers and forms and got all my rebate cheques

I didn't want to get denied because when I phoned into PF they said each MIR had to be in separate envelopes

its on 40% off this week at shoppers…I really wish it was a 20x weekend. I think I am going to take my chances and wait and see if it comes up again paired with a bonus points weekend.

I got some and used my personal emailed coupon for 20x wus 50$ and also got the 20$ gas card wus 75$

Thanks for posting various rebate forms in one thread, can any one confirm if they did apply rebate after using coupons and received successfully.

Help! I bought PF last weekend and after reading the tiny rebate coupon and realizing I did not need the UPC, I opened the package and threw it away. Now I'm filling out the form and it's asking me to write in the UPC. Has anyone kept theirs? I bought the BB All-in-1 Beauty Balm Compact Cream.

Just make sure you follow the instructions - form, upc and rebate sticker

Hi I got all my rebates yesterday.Today I went to shoppers and there were 3 different ones in clearance section with $9 off rip off stickers and $6,$8.$9 rebate stickers of 2014 on them. I went to beauty counter and asked why they kept $9off everything then she said someone opened the cover but preis unused. I purchased them and want to apply rebates to my aunts address. Don't no if it qualifies or not.
