I am very informal at work we are all on a first name bases and actually the first 5 years I was hear until it became mandatory I did not even have my last name in my email signature. I was trying to blend in many customers were shocked when I had to add it that I am as they put it "part of the family" no we are not mob just a family business.
As well it became nice when people started to know my name and I was not John's daughter or even better "the girl", a lot of them knew one of the girls joined the business but could not keep the family tree straight so I became "the girl" or "the daughter" or "her". So now that they even just know my first name it is nice.2022 is going to be my year, the year I find organization in my life and the year I focus on myself,

I'm a fairly formal person so I like using last names. When I'm in a close environment then I use first names but I have to be fairly familiar with the people.
One of my pet peeves is when people act like they know you when you've only met a few times. Drives me crazy.
I especially hate it in retail environments. I walked into Michaels the other day with my DD and the main cashier (I've talked to her while checking out, etc) ran up and started talking like we were friends. Commenting on my darker hair colour, etc. I kept thinking she must be confusing me with someone, but apparently not. She talked about DD and seemed to know more than I'd like. I felt very awkward.

I've rarely had to wear a name tag at work (several times they tried to implement them and it's failed every time.) One time this one irritating person went on and on saying my name. Then every time he came into the shop, the same thing. My opinion is that name tags are very rarely about good customer service and all about knowing who to blame when something goes wrong.
As I've gotten older, the people who I called Mr. & Mrs. are now greeted with their first names.

Well at school we tend to call each other 'miss' or 'sir' out of habit. It's all we hear all day. I catch myself doing it everywhere.

I guess I'm really informal... I can't think of anyone aside from previous elementary/high school teachers who I would call by their last names or address as Mr/Mrs.
I've always preferred to be called by my first name. (and I'm pretty sure my eye twiches when someone calls me Ma'am)
