There's a question on my income assistance form that i need help with. it's asking me if I have moved or enter facility, what should I write on there? (yes or no Q) i just started working 3 weeks ago.

That could be a question related to an assisted living accommodation like nursing home or hospice or adults with special needs supported by a team in a residence. It may also refer to a shelter if an applicant does not have housing but relies on community organizations for lodging/accommodation. The government just needs to know if there is a change of address or a move into shelter.
