3. Print your redemption form.

Costco must be the only place selling the HUGE bottles WITH the "specially marked" stickers.

I got it first try with UPC... it's already in the mail, lol.

Make sure, when entering UPC code, you do all #'s on the code, including the very first one and the very last one... No spaces.

I bought 16 bottles. (would have got them anyways for Xmas baskets!) I use my home address, work address and both my parents & in laws addresses!
