At least they did eventually get back to me :) But nearly 3 weeks is a long time to hear back from an email :P

I have not received the $15 GC, it has been over three months. Should I call "Dare" now or wait longer?

You might as well call them now to find out what's going on. The rebate was only supposed to take 6-8 weeks so 3 months is too long.

It asks for your info before letting you get the printable form, but it just gives you a blank form - generating a form filled with the info you gave would be so simple, I don't know why they didn't do that. So if you want to skip that step or just read all the fine print, the PDF is attached below.
