normally you would go to your hr dept and let them take care of it and fill out the papers for you to sign and bring to ei

I think you have to take it to your boss or HR and they will have to put you on Short term medical leave until your mat leave kicks in.
This way you will get paid for the 3 weeks ur gone before mat leave

I am not 100% sure but I think it is something like this
