hi,first i make the income tax by myself and i wrote the union an email(got no answer) and i called, the person only tells me, she had no idea, and the person who send this out is not available.
my hubby is in the union. last week he got a letter with a T4 slip. we never got money from the union....there was a information beside the T4 Slip:
The T4 you are receiving is the amount of life insurance premiums paid by the Trust Fund on behalf of the members and is considered a taxable benefit.
The government considers month covered by self -payments as a non-taxable benefit.This is because the member is paying towards the life insurance on his or her own behalf.
The monthly premium amount from January to June 2009 is 40$/month, and July to December 2009 38$/month.

Life insurance premiums paid by the employer are a taxable benefit. It's not income directly given to your husband, but paid by your employer for him.
You report the amount from box 28 on the T4 slip, on line 104 of the return -

hi, thanks. that helps. i was looking already but didn't find that.
now i have another problem. i can't find a line 104 in ufile.ca
(On line 104, enter other employment income.)
but there is nothing about this benefits for premiums,only following:
Labour adjustment benefits [Display help for this item.]
P13311 Maternity benefits [Display help for this item.]
P13080 Grants or training allowance [Display help for this item.]
P13546 Death benefit received from employer upon death of your spouse [Display help for this item.]
G143 T1198 - Qualifying retroactive lump-sum payment
P15004 Other taxable income (federal line 130) [Display help for this item.]
sorry, but it's the first time i have this thing.

life insurance premiums paid by the employer should be in box 40 other taxable allowances and benifits and box 14 employment income
