Canadian Wheat Board Cake Server mail in rebate (with $2 in postage)

In the booklet "The Love of Baking" there is a mail in rebate for a Cake Server from the Canadian Wheat Board. The booklet can be found in stores. A copy came with the November issue of Canadian Living.

The scan came out pretty blurry, so here are the important parts:

To receive your cake server by mail, complete the form below, include $2 in postage stamps (stamps must be sent in such a fashion that they can be affixed to an envelope) and mail to (address). Each offer must be mailed in a separate envelope, bearing sufficient prepaid postage. Offer limited to 5 requests per household. Request received without $2 in stamps or after January 21, 2011 will not be honoured nor receive a response. Expires January 14, 2011 or while supplies last. Allow 8 weeks for delivery.
