how do you decide where to place the comercials? and r u the one that says where they go?

Yes, its based on what the agencies buy for their clients, how long the spots are and what type of product they are advertising.

that's BAD BAD BAD!!!

If you see the same commercial at the beginning of the break and at the end of the break - those are called "bookends" and they are supposed to be like that. However, two of the same spots running back to back is either a scheduling error or Master Control asleep at the controls!!!
