When a coupon says " one coupon per perchase " does that mean one coupon per item or one coupon per transaction?? If I have 4 toothbrushes and 4 coupons, can I use one coupon per toothbrush in one transaction or do I have to divide them up into 4 separate transactions??

It's one coupon per item. Means you can't wear more than one coupon on one toothbrush.

One coupon per item. Although if you're lucky some cashiers might put another phe through!
