how to do a MIR for the lysol soap dispenser?

i have a MIR form for the lysol dispenser but i have no idea how to use it...do i mail it out with my receipt for the refund?

sorry but i am new at this and didn't know how to go about using it...

You have to mail the receipt with the form and the purchase price must be circled and the UPC handwritten on the receipt.

Wal-Mart has it on sale for 6.97. It would be better if you have the MIR. I found mine from Smartsource a few weeks ago.

french or english the address is still the same. just look where it starts 12243 thats your po box number, I'm sure you can figure out the city, province and postal code from there.

I saved my receipt but forgot to right down the UPC foe this MIR. Is there any way I can get the UPC somewhere? It just needs to be hand written on the receipt. I bought the cucumber scented no touch system. Let me know if this is possible.
