    A $5 Tim's gift card for doing a short survey? If it seems too good to be true, stay away. Way to fishy for me. People can do a lot with all of that personal information!

    And when I say personal information, I meant in response to Star Dancers post in another thread that read..

    Dear Consumer,

    Many thanks for contacting us and our sincere apologies that you were not able to successfully participate in the www.myurbanvote.com research initiative.

    Due to strict government regulations, we must ensure that everyone that is participating in the research is an adult in their province of residence which is why we use the Equifax credit check as an accurate age verification process.

    Having said that, please find below a list of troubleshooting tips in case there were issues with the server or if your information was not entered correctly.

    If you are having difficulties please note the following:

    CORRECT INFORMATION FOR AGE VERIFICATION: your name and address information must match the information that is listed on your credit rating. The Equifax system will identify you immediately if this data is a match. A good source for this information is your valid driver’s license or Canadian Passport. Please ensure all fields match exactly as they are listed in the entry form. If your address and name do not match your Equifax record, we will not be able to provide you with instant online age verification and you cannot participate in the online version of the survey. As of July 20, 2011, a message will alert you if your information is not correctly entered.

    SERVER ACCESS: occasionally, the server may be down and the age verification process will not be in place. If this is the case, please come back in a day or two to participate in the survey. As of July 20, 2011, a message will alert you if the server is down.

    PREVIOUS ENTRY: if you have already participated you will not be able to enter again. This will ensure our research is correct and we can determine the packaging design most voted for based on this. As of July 20, 2011, a message will alert you if you have already entered.

    If none of the above mentioned suggestions solve your technical problem you are welcome to enter manually. We have enclosed for your use a manual entry form. To enter please follow the mail-in instructions and we will gladly verify your age and conduct the survey over by mail.

    Once the documentation is filled in, it will then give you a chance to provide your feedback.


    Please fill in the attached age verification form and sign the declaration form. The filled in documentation should be mailed in with a photocopy of your valid driver’s license or Canadian Passport for manual age verification and the opportunity to participate in the www.myurbanvote.com research initiative.

    Once we have verified your identity we will contact you and send you a copy of the survey to fill out and mail back.

    How to participate:

    ·Fill in the age verification form
    ·Sign the declaration form
    ·Photocopy your valid Driver License or Canadian Passport
    ·Send the complete forms to:

    Newsflash,.... if they have enough information on you to do an Equifax credit check,.... then they'll have enough information on you to GET A CREDIT CARD UNDER YOUR NAME!!!

    Do the math,... it not worth it,....

    Yes it is still up they will not send gift cards until the survey has concluded. Who knows when the survey will be over.

    If you cal the number on the survey someone answers from JTI Macdonald.

    -I- Can't even get credit in my name.. LOL

    However it worries me that they are based out of Quebec.
