Did any of you have problems getting the proper form for this? My Sobeys would not display the forms and would only give me a photo copy. I spoke to HO about it, so the photo copy should work.

I used the form they had on the tear pads and still have not rec'd the gift card.

My m-i-l got me the form. As when I went to the store to get the deal no one knew about it, or where the form was. She went back later in the day to get it for me and it was a tear pad form...I said, thats it?
