It says you need the original mail in rebate form. Is this considered the original?

Yes. The offers have different P.O. Boxes (12499 and 12500) so they are separate MIRs.

Shopper's Voice just sent an email for a mail-in rebate on
Air Wick Scented Oil Starter Kit or Scent Ribbons Candle. Must make purchase between Nov 21/11 and Jan 21/12 and send in refund form off there site, along with dated cash register receipts with the UPC code recorded will be accepted. lt says one per individual, family or household. But you can purchase one of each as two different address to mail too. Will make a very nice Christmas gift for someone.:top:

Thanks, I printed them off and I will give it a try. I have been filling in those surveys for nine years and only twice have I ever gotten coupons from them. I did the most recent survey but never received this email.=(
