Thank you for writing this -- I really hope people answer this honestly and only once (that is don't do it numerous times to get free cards)...for people with familes with chronic and/or hereditary conditions this study should be able to provide some very interesting information, as long as its not taken over by people just answering whatever.

I had done the survey a while ago (after learning about it at a doctor's office) without getting a gift card. I really hope the gift card does not get people taking advantage. Please answer it honestly.

Grrr I REALLY wanted to do this one BUT I read carefully through all of the disclosures my medical records and medication I'm taking is a bit more information than I'm willing to give for a $10 giftcard. Great find though for those that have no problems!

It should ask right after your finished. should say "click here to choose your gc" In red writing.

You also get an email stating that you have until March 22nd to redeem your gift card and a link to click on to claim it.

Everything..your name , address , age , sex and of course your OHIP number..too much info I guess, for a $ 10 card.

You can opt to not give your OHIP number. You can pretty much opt out of all questions your not willing to answer except your name and address and phone number.
