I just tried and it told me 1 per every 12 months but I just retried with a different email bt all the same information and it went through.

Got it thanks! And it looks like the sample packs are based on e-mail addresses. I changed my e-mail address and I was able to get a sample.

I got one in 2012, and this is 2013 if I am not mistaken. Maybe they mean Calendar year? Like the Chapmans coupon.
