It seems to be working now - just submitted my request and got a page saying "Thanks! We're working on your oder. Thanks for taking the time to sign up. Your sample should be withing 6-8 weeks. Once you've had a chance to let it soak in, come back and tell us what you think on our Rate & Shop tab."
