Might be this one. It's from either a Smartsource or Redplum insert. I did the scan on April 17 '12 and I remember trading them all away.

$3 off any Physician's Formula cosmetic product, #90402359, expiring July 31 '12. :-)

***edit*** It is from the April 14 '12 Smartsource insert, with L'Oreal on the cover.
