Thanks nshoren. I did it on my laptop not knowing it's a print coupon, so I saved it as a screen shot, hope it works. Also, hope somewhere in Toronto & area sells it...

Heads up: when they ask if you have bought Silver hills in the past, say YES. If you say no, they decline you filling out the survey.
Then you would have to start over again
Darn it said it was closed for me.
I put Ontario and it said:
"Wow, we've already had an overwhelming response from our Canadian friends. Sad to say, the survey is now closed to Canadians. Don't worry, we love surveys and there will be more opportunities to provide feedback (and get free stuff) in the future."
I did the survey on my phone thinking it was going to be a mail to home coupon. :(

Try Alberta, I entered with AB and it seemed ok
Does anyone know which stores would accept an internet FPC?
Went to my local Co-op over lunch and they took the coupon no problem.

Depends on where you live, locally to me, only Loblaws banner stores will accept printed FPCs
Walmart's policy states they accept FPCs but do not list them as accepted under printed so likely do not
