    So my question with this promo I guess is does the form require a separate receipt? How long did you receive the cards after mailing them off? The reason why I ask is that I noticed next week Walmart has Energizer batteries on sale and maybe if I am lucky enough to find some with the promo still in them I can take advantage. Christmas is coming and that means lots of batteries!

    I sent a ton of energizer forms more than 2 months ago and I still haven't received anything how long did it take you to get your cards?? Thanks!

    Yes you should see it on the front of the package but it will say $5 gas card but it's really a $5 prepaid visa card that they send you

    All you need is the Form and the Upc from each battery pack. You will cut out each upc card and fill out the form and send them all in separate envelopes. Makes to read the directions

    I sent my first 9 on Sept 9 and my next 3 Sept 17 Sometime around Oct 15th I received x10 $5 prepaid visa cards. I am still waiting for 2 more. I think if you don't receive them soon you should probably phone Energizer.
