my walmart still had forms when I went there last week, check the end-cap of the aisle for a cardboard display of speed stick/lady speed stick stuff and the form pad should be attached. If you don't see it you might be out of luck though.
I also have one form left for trade that I can mail out ASAP.

I found my MIR form at Walmart near the deodorant - this was late May/early June.
Received my ticket today!

I didn't think this was going to come back to me because I used coupons on some of the products on the receipt I sent in, and they all showed up on the bottom of the receipt, but it finally came today!!
I LOVE the long expiry date on the movie pass!
