Claiming medical expenses on taxes...

I am using UFile to prepare taxes for 2015. Just wondering when claiming medical expenses do you have to list the name of doctor with his/her phone number and address or can you just list dates and type of medical expense?

Usually its the same doctor. I went to the dentist 6 times last year, and I was billed 6 different times.

I couldn't combine this info in Turbo TAx on 1 line. Had to do 6 lines. So I just copy pasted the name and the Clinic name on each line. And manually entered the amount and date on each line.

you have to put the name and/or just the clinic name.

Need not put the address and phone number.
