One UPC per household....Different shades different UPC's so you can claim all shades
1) Buy the products with rebate stickers attached to them. DO NOT USE COUPONS even if its for other products. Carol Robertson will not approve your rebates, even with a detailed explanation and she does not seem to know that Physians formula coupons are not circulating at the moment.
2) Print form from physiciansformula.com
3) Fill out form and attach rebate stickers, write down UPC's, circle product on receipt
4) Mail out
Will this work with the 20x event this weekend?

Sweet deal OP :) I haven't seen rebate stickers on any PF products in the pharmacies I went to, so I am not participating in this one. Has anyone else had the same experience?
I bought 4 bronzers and powder for $12.99 each had a $9.00 rebate for each oneAttachment 315394, which brings it down to $3.99 each, and I had a personal offer yesterday spend $50.00 get 12,000 pts ($15.00) which brings down to .25 cents each plus tax!!
