Doesn't seem to work in Internet Explorer but I tested it in Chrome and it works. You need to log into Facebook and then the form to fill out will appear.

Try the first link. The 2nd one doesn't work but if you click the first one using Chrome (Doesn't continue on in Firefox) then it asks me to log in with Facebook then the address form appears after you first have picked the sample you want.

There are still some available.

i just received my pack a couple of days ago.
