Does anyone know if it can be more than TWO boxes in one receipt? like 4 or 6 or 8 or 10 to reach maximum (5 requests). I read the faq but didn't find the answer.

Nope, only 2 boxes per receipt.
The FAQ says Each receipt can only qualify for one (1) $5 Kellogg’s* Grocery Prepaid Card and it can only be submitted once.

Has anyone had a hard time redeeming these? Cashiers have told me multiple times they "don't think we accept those" despite accepting Visa?

I got my Visa card a while back and had no problems redeeming it, they just treated the $5 of my bill as a Credit purchase putting in $5, and the rest I paid with another card from memory. They should not have any trouble accepting them, since it is a Visa anyhow
