    I want to have some kind of advance directive written up so as remind my parent not to call 911 when I pass but instead call a funeral home.
    Simply don't want resuscitation when I pass away.

    Tonight while looking for such a form, I found the Dying with Dignity Canada website. Two days ago, it was Advance Care Planning day! April is also Advance Care Planning Month.

    There's an Advance Care Plan kit (free pdf download) option for every province and territory. Thought to post the link in case another member of the forum is reviewing their own advance care plans or are in the process of discussing such matters with other people.

    @Ciel , I am not sure if you have a family doctor or if yours offers this but when my Oma passed my Opa had been instructed to call the family doctor, not 911 for exactly this reason. The family doctor then took care of everything and when she was done called the funeral home.

    I have understood for some years that besides a doctor, a funeral director can sign the death certificate (after talking to a coroner about cause of death) in Ontario.

    I never got a different family doctor after being told a couple of years ago I could not keep using the place where I had gone. For those of you with a compassionate/with-it doctor who is familiar with how death certificates work, you have a good person on your care team. Not all doctors keep up with current medical issues/care needs/etc especially if it means more paperwork for the practices.
