At first it said "No longer available". But I copied the link and opened it in another browser (Firefox) and I was able to fill in the short survey and order the samples. YMMV

@sesterce I tried it in both Firefox and Chrome and got the same " no longer available " message.
Three times I tried it on each browser and in FF I would get this message for a split second, then it would switch to not available.
I sneakily screen grabbed the image as it was there for that split second....lol!

@sesterce i was able to order sample by pasting the link on Firefox browser. Thank you!
