Name - Is the name that will appear on you comment. If you look at the first comment there, "Georgia" is what the first commenter wrote in that field. You don't have to put your full name, or your real name.
Mail - put your real email address. If you win, this is how you will be contacted. Only the website administrators will see your email address, the public will not see it.
Website - if you have a blog or another website, you can put in the website address (consider it free advertising). If not, you can leave it blank. Most people leave it blank.
