I don't thin kthey need the eye exam reciept, just the "official receipt"
12 boxes - 7 flaps
8 boxes you need to send in 5 flaps
6 boxes - 4 flaps
4 boxes -3 flaps
2 boxes - 2 flaps.
you get $75 bucks back for 8 boxes with Oasis for astigmatism.
I just got the info off the rebate certificate my optometrist gave me, so i don't know if it's completely different from fleetingleaf's post.

Hmmm, I surfed to the Canadian website from the link above and am having trouble finding a Canadian rebate offer. Am I blind or are we getting forgotten again? :S

I odn't know if they're still offering it. I just saw the certificate nd said it was good through Jan, 2008 to Dec 31 2008.
It's feb 09, i bought my contacts in November --' I don't know if i can still send it in now and i can't find a number i can call and ask.

i think that site if for U.S.
the canadian rebate can be found instore...they should give it to you with your contacts! (2009 rebate)

I'm not sure if they're doing it in 2009... although they have done it every other year (2006, 2007, 2008 - these are the years I've done it and gotten the rebate). I usually get a coupon from my contact lens store and mail it in before the end of that year, and have always gotten the rebate.
P.S. Biomedics offers a rebate usually as well - I also got a rebate form from my contact lens store and received the rebate quickly.Last edited by CSLT; Fri, Feb 6th, 2009 at 02:08 AM.

Wow, so costco isn't actually getting me a deal with the $50 MIR - I never heard of that anywhere else! I think their prices are still lower than other places I've bought lenses, so I'll continue to go there... just interesting to know!
