Do you mean the form? The form was in the flyer insert from smart source or redplum a few months back. It's not an online form unfortunately.

which Airwick was it for? I've had all of them but haven't seen the MIR.. That or I already submitted it and forgot (which is possible haha)

The most recent Air Wick rebate (ending Nov 30) is only valid in Ontario and Quebec.
