I don't see why not - it's right from Glade.ca and nowhere does it say you need to mail in one found in stores. In the thread about the rebate, someone did call Glade to ask and was told they could use the one on the website: http://forum.smartcanucks.ca/1414021-post205.html

If you think about it, what would be the point of having the form on the website if it can't be used - wouldn't it have VOID through it or something if it couldn't be used?
