Do I have to cut and attatch the bar code with the MIR form together?

I bought the Eyeliner and Mascara on sale for 12.99 at SDM, regular price was 17.99.

I previously bought the correction powder for 12.99 on sale at SDM (40% off), but I've seen it advertised for 10.49 at RCSS.

The rebate chque comes back in your full name that you put when you fill out the rebate form. Besides Rexall, you can find PF products at Shopper's Drug Mart (i think its called PharmaPrix in QC?), Walmart, etc. Great program! I hope they do it again next year! There was also a $3 off coupon in the Smartsource a few weeks ago (i found mine in the Toronto Star but not in my neighboorhood paper) so can't wait to use it![/QUOTE]

U have to write the bar code on the form that u will be submitting.

No need to send in a cut out.

U need the form, the try it free sticker and your original receipt. Mail all three together.

Personally I will be cutting out the barcode and sending it in since SDM receipts don't show the UPC on them

I got the eyeliner with serum (black and brown) and it is awesome! The best eyeliner I've ever used. I also got the mascara but I have yet to try it since I have so many already in circulation..

I can send them all in ( in separate envies of course) and use the SAME return address (address where the check is mailed to) on all of them? TIA

Be careful not to grab one that expired! A couple weeks ago and having a heck of a time contacting them to get this sorted, the number on the rebate forms can't help me they redirected me to another number 1.800 that doesnt work!

Only one rebate per product per household, so I would recommend sending one to another address since they are both eyeliners.
