I found the forms today at Shoppers. Here is a picture:
I wonder if there are any passes left? There should be a web site for this so they can show how many are left.

I contacted them about passes ( if they are still available and is it worth to mail upc and how many passes left)

My tickets came today!! They are for Empire Theatres and they are valid until Jan 31, 2013!
Luckily we have an Empire in Hamilton, unfortunately it is in Jackson Square and I hate going down there...oh well, free movie!

I got mine yesterday!
The pass I got is for AMC Theatres, which I'm glad they sent that instead of Empire Theatres because the closest one is over 40 minutes away.
