I asked the customer service people at my Loblaws and they had no idea what's going on. So, I've sent an e-mail to head office asking for copies of the form, or an online PDF version.

I'll post if they (ever) respond!

Went to RCSS here in Chatham ON but no Quaker forms! If anyone has for trade, I'd like 4 or 6 of them (some for mom, sil, sister)...I did find the new Kraft $7.50 booklets tho..got lots for trades!

I was at RCSS in St. Catharines this afternoon and they had lots of them. There are several pads on a display near the front entrance, just before the produce section. There's another pad at the end of the dairy aisle in front of the extra-large jugs of Tropicana.

My RCSS in Manitoba had no idea what I was talking about and I couldn't find the forms anywhere in the store. Guess I'll have to trade for them!

I asked the customer service people at my Loblaws at Queens Quay and they said the MIR was up for 2 week and now all gone, the display is still there but the pad is empty.
