The Similac Club.
You get a starter package with a couple cheques and some coupons. APPARENTLY, they are supposed to send out cheques to you every so often. But I haven't received anything since my starter package, so who knows

I don't remember filling out anything but I got similar cheques coupons a can of formula and a bottle of similac mom, it would be nice if I had a kid lol
