When they hit 3000 fans they will put up a form to be filled out but I don't think they send out 3000 boxes so you have to watch for it.Quote:

I kinda think I remember from the first give away they had it was for both Canada and the US.
When you filled in the form and then submitted it, did it just bring you back to a blank form?
