hmmm... so I just printed it. No where it says that its valid in Canada or US only. However the mailing address for the stores for refund is of US. Does it mean its not valid in Canada?
Will try to use it if I find the product in store.
Also, the expiry date is 8/16/2012.

If it has a USA redemptions address, it's for the USA. If you try redeem it in Canada, most retailers will reject, even if it doesn't say where it is valid as they don't get paid from USA redemption addresses.
