    Since I was diagnosed with type 2 diabetes in late January, I have kept my results on a spreadsheet. If anyone out there wants to use it, feel free to do so and if you do use it, feedback on improvements is always welcome. There are 2 for 2011 and I have tweaked them for 2012 also.

    I also saved it in the 97-2003 format with -xls at the end.

    Very nice, I like it! Quick question since I don't use Excel, can I take this current grid as it is and add in extra times/spaces? I take my sugars 6 times a day (blah) so I would like to tweak it for myself.

    Sure, I can do anything you want. Let me know what you want added and I will do it. I will repost a new one when I get back from work as I made a few adjustments to it in regards to the Year to Date formulas. (those are long!) After that I can tweak it to meet your specific requirements. It shouldn't take much time to tweak it just the way you want. There are 7 different time/reading slots and I can rename them to suit yours easily.

    At the top where I put a place for the name, I also put my file # that I have with my doctor so I can print it out and just hand it to her. They won't be mistaken for any one else's readings that way.

    If you can think of any other things to add or improve, let me know and I will see what I can do.

    Oh DH666, that's too kind of you! I didn't mean that I expected you to tweak it for me, I just was curious how easily it is to do it so that I can tweak it for myself. I'm just really bad at anything that isn't Microsoft Word, and even then it's touch and go LOL

    I do need to step out to meet a realtor and make my Mom go to the Drs (oh the joys of being an adult sometimes) lol so when I return I will look over your template again and see if there were any things that in general would be good to add.

    I really don't have any suggestions in changing your template as it covers the basics except maybe adding a spot at breakfast/lunch/dinner to write in what you ate at those times. I guess that would be valuable dependent upon how closely you monitor what you are eating and the interactions of your food/medication.

    I actually just took a closer look at your template *insert facepalm here* and see that you have the exact time frames that I take my glucose levels *duh* lol I take my glucose immediately upon waking up, 1 hr after breakfast, before lunch/1 hr after lunch, before dinner/1 hr after dinner, before bed. So in total I'm taking my sugars 7 times a day. It is a pain in the butt however being Diabetic for 13 years and nothing ever working I've found the only thing that does work is, well the proper medication which I'm now on and complete vigilance.

    I just noticed this a few days ago. It`s awesome. I used to write down my blood sugar levels but I love that this keeps track of the average. It`s great to have an idea of what my A1C will be when I get my bloodwork done. Thank you so much.
