    I called Physician's Formula's customer service and they said it was fine to buy numerous things on one receipt and send them in all together! Took advantage of the 30% off this weekend and 20x the points at Shoppers....the obly thing they did'nt have that I wamted was the mascara !!

    I sent mine in two days before the expiry date last November and I got my rebate cheques 2 days ago...so don't worry they are coming.

    Thanks for the info. I just printed the forms and noticed that different product has different mail-to address, if all forms, stickers and one receipt are put into one envelop, what address should it be sent to?

    me either ,only recall seeing rebate stickers at walmart but even if they don't have rebate sticker think i'll seriously consider this one!Depends how much and what products.
