Thank you for taking a few minutes to answer our questions.
This consultation survey is for people age 15 and over and is, open to both Desjardins members and non-members. Your answers are important and will be used for Desjardins's next youth-oriented action plan.
Answer 5 questions and fill in the fields about the respondent and you could win one of the following prizes, which all have a total value of approximately $10,000:

• 1 home theatre system with 42” TV
• 2 laptop computers with carrying case
• 4 digital video cameras with carrying case
• 8 iPod nanos*, 16 Gb with case
• 12 gift certificates for the Admission Network Inc. (Quebec) or Ticketmaster® (Ontario), valued at $150 each
• 30 webcams

One entry per person (age 15 and over).
