But you have to find it in the store. I bought 3 PF products yesterday, so if anyone knows where to find the rebate form, I would really love to know.

yes, you should be able to allow to use coupons. on the receipt it doens't say what the coupon is for which product. i've done many MIR before and i have always used coupons.

Physician's Formula is on sale at shoppers for 40% (...something like that) off this week, though i don't know when it ends. i picked up a bronzer for $12. Beware of the fine print on the $6 rebates!! they state that you have to include the little MAIL IN REBATE label attached to the product!!

I went back into SDM tonight to try and look at the different products and for the rebate form again. I did find the little rebate form. It's really tiny and attached right to the product. I am not surprised that I missed it the first time! And it is only on certain products. I bought the powder palette light bronzer (multi-colour bronzer). It had $6 off and looked just like the one posted in the pic above. The bronzer was $11.99 so it's like another 50% off.

I have a few of the $5 mail in rebate's for trade if anyone is looking. It is not the same as the $6 this was not attached to the product and is valid on everything except trial sizes, promotional items nor kits. exp apr 15 2010

I found these about 1 month ago at rexall and SDM

I bought 2 bronzers at SDM today, on the same bill the thing is, the rebate is one per household and you have to send in the original bill. What am I supposed to do? I was going to claim the second one in my cousins name just to get the money back but what should i do about the bill because i used a coupon so if i return it and get another bill i would end up paying more. Does anyone know how to go about this?

well since i can't find the rebate forms i decided to email PF and see what their respone was. today i got a reply and they said:

Hello Cindy,

Thank you for your interest in the Physicians Formula Rebates.

Rebate claim forms for this particular rebate are available at Shoppers Drug Mart, and should be kept near the Physicians Formula products. They look like pads of coupons that say “Physicians Formula $5 Mail-In-Rebate” on the front.
If you are unable to find these claim forms in your local Shoppers Drug Mart please print your full name, address, city, province, postal code, telephone number and email address on a plain piece of paper and mail it to:

Physicians Formula $5 Rebate
P.O. Box 250-PF4
Pickering, ON, L1V 2R4

Along with your contact information, please send the original UPC (barcode) from the product package and the original, dated cash register receipt with the purchase price circled.

Please note there is a limit of one rebate per household.
