There are about 5 pages of questions enclosed with the delivered product (also a 3 digit number on the label that identifies which concoction you've been sent). Most of the 5 pages are scales (on a scale of 1 to 10, how do you rate x characteristic). You fill this out after you've sampled the product, then TNS phones you back within a week and asks you to report your answers with them over the phone. Most product tests involve two variations of the same product, so after you have reported on the first product that was delivered to you, they send you a second, you repeat the process, they call you back etc. Phone call takes about ten minutes.
