A simple will is pretty easy to do . But a lawyer will be generally needed if your case is a bit complicated .
Also if you die without a will , Succession Law will go into effect & the property will be distributed by the gov. according to that . The law is pretty fair & if its a simple case usually most people are happy with the distribution , although your specific wishes are not followed . Wills and Power of Attorneys do not need to be notarized. They simply have to be witnessed by two witnesses who are not associated with you. For example, your spouse could not witness your will and any of your beneficiaries could not witness your will.
It is highly recommended you select an estate trustee and a substitute estate trustee in the event your first choice is no longer alive or is unable or does not agree to act.

I would think of two obvious ways to indicate DNR:
1) wearing a tattoo, bracelet or necklace tag with the Do Not Resuscitate words;
2) additionally, having that request put into your OHIP record at your regional OHIP office, so any health care worker seeing (1) can get a colleague to check the record to verify that request. I don't know for sure how a DNR order is registered, but getting it on the health record would make sense.
BTW, do you want that DNR applicable in any situation or just if you happen to be in a certain state or medical condition that might worsen while unconscious and trigger a drop in your vital signs and prompt staff to respond to the equipment alarm?
Another BTW-if you have made a choice to donate organs, you may have to see who is the next of kin that can approve that decision when asked by a doctor/health professional and if the DNR order would be permitted or not (overridden) in order to keep the organs viable for removal and transplant. http://www.giftoflife.on.ca/en/
For the legal aspect of the paperwork, there could be a service group that might offer low-cost legal help to the community.

ADMINISTERING ESTATES
The OPGT protects the interests of potential heirs when an Ontario resident dies leaving an estate and there is no one who can administer it.
The OPGT will apply to be appointed estate trustee if:- the deceased was an Ontario resident or owned real estate here; and
- the deceased did not make a Will or the deceased did make a Will but the executor has since died or become incapable; and
- there are no known next-of-kin living in Ontario or the next-of-kin are minors or mentally incapable adults; and
- the estate is valued at a minimum of $10,000.00 after payment of the funeral and all debts owing by the estate.

I'm aware that executors have to apply to CRA be released from their duties when they think they have wrapped up the legal and income tax aspects of the estate of the deceased. Otherwise, CRA can hold executors personally/financially liable to handle matters still ongoing with the estate. So that is another part of your paperwork discussion that needs consideration with whomever you designate as executor/co-executors.
