    Hi, how do I get an official document from ODSP detailing my income and benefits in the past 12 months (since Nov 2013)?

    Ontario Disability Support Program. It's the provincial portion of the disability program (the federal portion is CPP-D).

    I'm also on ODSP, not sure if I can answer your question, but I'll try.

    The mailing you get every month should have a sheet with 4 portions (dental/drug/statement of assistance/cheque (or direct deposit info). The statement of assistance should have the info you're looking for for that month. If you are ocd like me, you will have kept the statements and the info is on there. If not, call the caseworker to get the info. I believe they are in the process of automating a few things, so you should have access to this info online soon (not sure how it's being rolled out, whether it's gradual or for everyone at once). If nothing else, the info for the previous year (Nov/Dec 2013) should be part of your last tax return.
